Evaluate

• Assess the patient's needs and goals.
• Formulate a treatment plan.

Treat

• Active rehabilitation guided by a physical therapist.
• Progress recorded and provided to the patient's treating physician.

Transition Program

• Post insurance; continued recovery program.
• 30 day program, free of charge.
• Overseen by Physical Therapists and Cooper Certified Trainers.
• Continued monthly enrollment for about the cost of one insurance co-pay.
• Necessary program to maximize recovery.

Discharge

• Return to your daily active lifestyle.
